Difference Between Bookmarks with Page and Anchor Icons


hi,

 

i use indesign every year update large guide.  i've had sort of teach myself how use software, since don't use year-round, forget things.  1 of them has bookmarks.  i've noticed of bookmarks have page icon, 1 has anchor icon, this:

first of all, difference between these?   , secondly, whenever update table of contents, anchor moves have nested way top of bookmarks, right underneath table of contents, so:

does know why happening?  think has fact particular section isn't explicitly listed in table of contents, want have bookmark it.  nest bookmark in proper position, displayed in first image of post.  me out this?  thank you!

the 2 symbols designate different destinations. little anchor indicates destination text anchor, page icon shows destination page.

 

why anchor sorts way when update table of contents isn't clear. entirely random. update toc number of times in quick succession without changing in document. it's anchor turns in different spot every time update toc. that's kind of behaviour i've seen few times.
