what software is needed to create signable form for web app?

i build web application user presented ability open pdf file in adobe reader, fill in name/title , date, , electronically sign pdf.  when complete, there should button resulting pdf data/signature posts https site (and ideally close reader window when successful).  1 pdf, , few users start.  don't need extract data on end, want signed pdf.  adobe software need have able create pdf filled/signed in recent versions of adobe reader, , need whatever reader extensions is?

would inform may use acrobat pro dc creating such forms. may apply 'submit' button user submit form after filling fillable fields.before sharing the form have save form reader extended feature reader program users may fill form.
