Barcodes disappear in Print Preview, Printing, and then form itself

we need print this irs form , must have barcodes. problem happens on company pcs, running windows 7 professional, , on printer.
happens if use adobe reader 11 , adobe reader acrobat dc; tried updating reader, each of message saying we're on latest version. got rid of dc on 2 computers based on in forum saying caused problem, installing 11 didn't help. date on windows updates , print drivers.

 

here's happens:

• we open form , fine.
• we click on print form button. no matter printer set default, barcode disappear , become gray box.
• we press on cancel , barcodes gone form itself.

 

i tried "print image" since listed solution in forum, , didn't work.

 

any ideas?

try print form , see if prints okay.
