PDF Form and WordPress Website

how can upload pdf form on wordpress website, allow users fill in forms, , have them submit forms me (re-directed them email address), want receive them pdf file, , not raw data file?  know there way, since have fill many forms, online, , received copies of forms, filled, , had capabilities of either print or save them computer.  can me?  thank you.

1. upload site , add link it, other media file.

2. add button, go properties , add submit form action under mouse trigger.

under target url enter "mailto:me@server.com", using own email address of course, , type of file submit select submit complete pdf.
