Interactive images within a pdf

greetings.

 

i've got physical therapy office looking make intake forms fillable - that's simple part isn't funny.  here's problem:

 

on intake forms, have drawing of body , request client circle area of pain , indicate type of pain in area, x sharp, + tingling, etc.

 

has done before?  do-able?
